Checklist for New Student Enrollment:

Parent needs to bring the following to enroll their student:

1. Name of school withdrawing from with address and phone number (fax number if able…not all forms that come to me have this information on them)

2. Official Withdrawal Form (departing grades should be on the withdrawal form if they leave anytime in a semester; it helps the teachers and counselors)

3. Transcript

4. Birth Certificate

5. Immunization Record

AUHS requires the following forms to be filled out before attending classes.

1. Registration Form

2. Race/Ethnicity Form

Immunizations required by Arizona Revised Statutes 15-871 – 874:

1. Tdap: 1 dose when 10 years have passed since the last DTaP, DTP, DT, or Td.

2. Meningococcal: 1 dose recommended.

3. Polio: 4 doses meet the requirement even if all 4 doses were given in the first year of life.

4. MMR: 2 doses

5. Hepatitis B: 3 doses

6. Varicella: Varicella vaccination or history of chicken pox disease.

Arizona law allows exemptions; exemption forms are available from the school.

If you believe that you, your child, or a child in your care or custody is experiencing homelessness, AUHSD is here to support you. Many of the typical enrollment requirements are not applicable to someone experiencing homelessness and we will make sure that you or your child are enrolled the same day and that we discuss additional resources available to you ASAP.
